Let's delve into some of the most common medical causes of male infertility-

Varicocele

A varicocele is an enlargement of the veins within the scrotum, and it is quite similar to varicose veins in the legs.

Impact on fertility:

The temperature around the testes rises due to the dilated veins and has its interferes with sperm production while reducing its quality. This is found among 40% of men as this is the most common and primary cause of male infertility.

Diabetes Mellitus
What it is:

Your sperm DNA can also get damaged due to chronic high glucose levels, which reduces the potential for fertility.

Hypogonadotropic Hypogonadism
What it is:

Hypogonadotropic Hypogonadism is one of the hormonal disorders where your pituitary glands don't make the production of enough gonadotropins, namely FSH (follicle-stimulating hormone) and LH (luteinizing hormone)

Chronic Systemic Illnesses- Kidney or Liver Disease
What it is:

Under this, the hormone associated with milk production, known as prolactin, is elevated due to a pituitary adenoma.

Impact on fertility:

How to Prevent Male Infertility

Male infertility can be prevented by maintaining overall reproductive health and avoiding certain factors that could damage the function and production of sperm. Some of key preventive measures are-

  • Should have a balanced and nutritious diet rich in antioxidants like vitamin C, E, zinc, and various essential nutrients that support sperm health and production.
  • Should avoid high-intensity workouts as they impact fertility negatively, and exercise moderately to boost testosterone and semen quality.
  • Stress needs to be managed effectively with the help of relaxation techniques such as exercise, counselling, and meditation, as chronic stress as help reduce testosterone and sperm quality.
  • Another thing that you must ensure that avoid smoking, illicit drugs, and excessive alcohol, which impairs sperm motility and its count.
  • Limit your exposure to environmental toxins and heat, such as saunas, hot tubs, and tight clothing, as it increases scrotal temperature, which harms the production of sperm.
  • Try ot maintain a healthy bodyweight while avoiding hormonal imbalances as it affects fertility.
  • Prevent and promptly treat infections of the reproductive tract, including sexually transmitted infections and mumps orchitis, which can cause permanent damage to fertility.
  • Avoid the use of harmful medications or steroids unless prescribed and monitored by a doctor, as some drugs impair sperm production.
  • Early correction of congenital or acquired conditions like cryptorchidism (undescended testicles) and varicocele can prevent future infertility.

Diagnosis of Male Fertility

Diagnosis involves a stepwise approach:

  • Assessing medical history and physical examination has its focus on past illness, sexual function, surgeries, and lifestyle factors.
  • Analysis of semen is driven to assess motility, volume, sperm count, and morphology, it may require repeated tests to analyse its accuracy.
  • Hormonal testing (testosterone, FSH, LH, prolactin) to detect endocrine causes.
  • If indicated, genetic testing is needed, especially in those cases where azoospermia or severe oligospermia takes place.
  • Imaging studies like scrotal ultrasound to detect varicocele or testicular abnormalities.
  • Specialized tests, such as testicular biopsy or sperm function tests in complex cases.

Treatment Pathways for Male Fertility

Treatment depends on the underlying cause:

  • Some of the required modifications in lifestyle such a balanced diet, regular exercise, and avoidance of harmful habits.
  • Can opt for medical therapies such as hormonal treatments (HCG, FSH, GnRH) for hypogonadism or other endocrine disorders.
  • Surgical interventions like varicocelectomy, correction of obstructive azoospermia, or vasectomy reversal.
  • Assisted reproductive technologies (ART), including intrauterine insemination (IUI), in vitro fertilization (IVF), and intracytoplasmic sperm injection (ICSI) when natural conception is not possible.
  • Use of donor sperm or adoption may be considered in untreatable cases.
  • Psychological support and counseling to help couples cope with the emotional impact of infertility.

Prevention leads to management of your lifestyle in such a way that any condition affecting your fertility could be managed.

Diagnosis is comprehensive, and treatment is personalized, ranging from lifestyle changes to advanced reproductive technologies depending on the cause and severity of infertility.

dr. nisha bhatnagar is next to GOD to us .We had 2 failed IVF attempts from ludhiana(2018) & canada (2021) consisting 3 embryo transfers in which 3 A grade embryos transferred in june,2018, 4AA,3AB (JAN.2021) resulting in chemical pregnancy & single EBL in june,2021 .We were helpless, exhausted, sad, depressed, highly frustrated ,worried, anxious as we had spent 5 yrs , too much money$$$ on private IVF's with no success. We packed our canadian life in boxes with great inconvenience & came to delhi . we saw few fertility specialists & selected dr. nisha bhatnagar after doing our research ( nov.2021- 2022) .we left the msg on the website & we received a call back next day & 1st meet & greet appointment was booked which was free & dr. nisha went thru ' my previous long Rx reports, tests ,results so effieciently .quickly ,thoroughly . She sounded so confident ,positive ,took great interest in my lowsy case ,did on the spot ultrasound ,answered all my questions with direct eye to eye contact & gave us HOPE .On her advice , I got ERA Test done ,got our frozen leftover embryo lying at ludhiana clinic from 2018 IVF Rx ,aveya provided us free NITROCAN to bring it & gave us all the needed support. I had 2 failed embryo transfers in a row at AVEYA ,feb 2022 in which 5 BB frozen ludhiana self egg embryo was transferred by dr. nisha with the help of HARSHITH (embryologist) ) but beta HcG TEST was negative . Then, in March, 2 donor egg embryo's 4AB & 3BB were transferred with no result. I still remember ,I was broken,SHATTERED & crying in pain as lots lots of effort in terms of medicines, time ,money is put into each cycle .Dr. nisha said so many motivating words that, don't worry , you are my responsibility now, we will try again . She helped me when i needed it the most . She advised me to use fresh donor eggs , A.I. SOFTWARE to pick sperms for ICSI , get expensive PGS test done for some embryo's to rule out chromosomal abnormalities (mosaicism) for next cycle, to which I agreed with no fuss. Another ICSI is done with a new donor (with high known reproductive potential ) & good # of embryo's were formed to our relief . One single blastocyst non -PGS tested 5AA was transferred in april & now, have a beautiful healthy baby boy ,born in canada as canadian citizen . i loved that dr. nisha is available all the time on her what app 7am to 12 midnyt. which is exceptional . She is super active, knowledgeable ,kind ,energetic, ultra confident ,positive ,multi tasker , she runs the whole show at aveya single handedly .she is fast & always busy. She modified the Rx plan from time to time ,as I also had few suggestions ,doubts, apprehensions. She always listened to me .I was able to discuss my doubts, myths, omens, worries, suggestions, questions .She never got angry. She had full confidence that my uterus is okay ,looking at ultrasound findings when i & my husband had doubts about uterus. She never pushed us towards surrogacy for greed of money . Aveya's pharmacy & billing deptt. is very friendly & efficient . On site blood collection & sample collection for various tests is worth mentioning . RX cost packages are very reasonable . Reception is friendly & runs smoothly .Their in- house embryologist is polite ,young dedicated professional who co-ordinates well with Dr. nisha. She really likes & trusts harshith . dr. nisha is a dedicated true professional , expert in her field , ,who is caring ,compassionate ,competitive who devotes the maximum time possible to her job, patients as she is herself a mother of 2 daughters .Her team is good . If you have a complicated case, past multiple failures, who have advanced maternal age, have no hope , limited budget or want instant success, want a customized personal attention ,I 'ld recommend DR. NISHA BHATNAGAR working full time for aveya fertility. I had a very very very positive happy nice super good once in a lifetime experience with dr. nisha /aveya . THANK YOU !
